周围神经战斗损伤的神经外科护理方案

在现代武装冲突中,上肢和下肢的枪伤和地雷爆炸伤占所有其他局部损伤的首位。在以往的武装冲突中,周围神经重建手术干预时间不早于3-6个月,损伤后,因为功能障碍通常与挫伤机制有关,并且有可能无需手术治疗恢复神经功能。为了遭受周围神经损伤的伤员,作者制定和推广一种新的护理方案。近年来,磁共振神经成像和周围神经超声检查等神经影像诊断技术在军事医学院得到了广泛应用。通过这些技术的应用,可以确认或者排除神经干的完全解剖学断裂。在存在特征性病理形态学变化的情况下,将对周围神经进行重建手术干预,现在这种手术在受伤后(1~1.5个月)尽早进行。越早对神经进行重建干预,其功能恢复就越快越完全。周围神经损伤伤员护理的新方案,要求神经外科医生和相关专家之间采取综合方法、密切互动。这样就有可能在接受外科手术的病人中发现周围神经系统病变的病例。这种方法在治疗此类伤员的神经性疼痛方面已显示出效果。及时进行重建干预,可以提高肢体丧失功能恢复的效率和速度,从而缩短康复周期,提高生活质量。
