骨代谢实验室标志物在机体适应储备状态及运动系统障碍中的作用

详细
论证。多种骨组织代谢标志物的功能在机体适应不同性质应激负荷的过程中发挥着重要作用。
目的。探讨骨代谢实验室标志物在Federal Fire Service of the State Fire Service of the Ministry of Emergency Situations工作人员机体适应储备状态及运动系统功能障碍中的作用。
方法。对105名隶属于Federal Fire Service of the State Fire Service of the Ministry of Emergency Situations of the Russian Federation的男性工作人员(平均年龄34.7±0.4岁)进行健康随访观察。根据运动系统状况将受试者分为三组,并根据职业工龄分为三组。采用封闭式化学发光免疫分析仪Immulite XPi(Siemens,美国)测定血清甲状旁腺激素、胰岛素和脱氢表雄酮硫酸酯水平;采用封闭式化学发光免疫分析仪Access 2(Beckman Coulter,美国)测定血清皮质醇及尿液去氧吡啶啉水平。血清骨钙素浓度采用酶联免疫吸附法(IDS N-MID Osteocalcin)测定。计算脱氢表雄酮硫酸酯/皮质醇指数以客观评估机体适应储备状态,同时计算胰岛素抵抗指数HOMA-IR及骨钙素/胰岛素指数。
结果。在所有研究组中,各项检测指标的平均值均位于参考范围内。与健康受试者相比,中度运动系统障碍人群的骨钙素水平降低(10.89±0.56 ng/ml),其在健康受试者中的平均水平为(17.21±1.33 ng/ml)。在职业工龄≤5年的人群中,骨钙素平均水平(18.50±2.15 ng/ml)与参考范围中位值(19.8 ng/ml)相当,并与工龄≥15年人群中较低的该蛋白浓度存在差异。适应储备处于主动消耗状态的受试者,其骨钙素浓度高于适应储备保持者。随着职业工龄的增加以及在存在运动系统障碍者中,骨钙素/胰岛素指数呈下降趋势。
结论。对Federal Fire Service of the State Fire Service of the Ministry of Emergency Situations of the Russian Federation工作人员进行的综合实验室检测结果表明,其骨代谢标志物水平未发现病理性改变(即未超出人群参考范围),不足以用于诊断临床明确的运动系统障碍。然而,所得结果证实了运动系统在机体多系统应激适应中的参与,并确定了在危险职业人群中,进一步研究骨钙素及骨钙素/胰岛素指数在运动系统疾病早期诊断及职业负荷耐受性维持方面作用的合理性。
