中风后患者尿失禁作为医学社会问题

背景。尿液排泄功能障碍,特别是中风患者的神经源性下尿路功能障碍,是一个重要且需要医学界高度关注的问题。
研究目的。确定因神经源性排尿障碍引起的尿液排泄功能障碍在中风后患者中的流行情况,并评估尿失禁对患者生活功能的影响。
材料与方法。分析了88例中风后患者在神经外科康复科第二阶段康复治疗的住院情况,患者的康复分级量表得分为3–5分。不包括因认知功能严重受损而无法评估排尿情况的患者。通过问卷调查(ICIQ-SF、FIM、FAS、SF-36)评估和确定排尿功能障碍对医学社会领域的影响。
结果。在急性脑血管意外后患者(康复分级量表得分3–5分)中,46.6%的病例出现了排尿功能障碍。研究发现,在中风后超过三年的患者中,排尿障碍的发生率增加。排尿障碍患者更频繁地需要他人协助控制括约肌。排尿障碍对中风后患者的生活质量和疲劳水平产生了显著影响。
结论。识别中风后患者的排尿功能障碍并将其纳入综合康复计划,以及开展相关的功能补偿措施,是康复治疗中不可或缺的一部分。
