不同侧化表型孕妇在不同压力水平下的激素状态特点

详细
论证。长期居住在战区的孕妇在调节过程的适应性特征方面具有较高的压力易感性,这一特殊人群的研究具有重要的科学意义。女性机体的形态功能不对称性(侧化表型)是影响压力耐受性的重要体质特征,其中神经和体液调节环路在压力耐受性的形成中起主导作用。
目的。探讨长期居住在战区的孕妇在不同压力水平下的激素状态特点,并分析其与侧化体质类型的关系。
材料与方法。设计:本研究为前瞻性、抽样、比较性研究,采用非随机分层全群抽样,纳入年龄18–28岁、首次单胎妊娠且妊娠过程无并发症的孕妇,这些孕妇在临床、激素、生化、超声及多普勒检查中均未发现产科病理表现,并且在孕前已在相应地区连续居住不少于三年。研究内容包括侧化表型鉴定、压力水平评估、循环系统适应潜能、情境性与特质性焦虑、激素谱、血常规及凝血功能参数测定。
结果。研究发现,右侧化表型孕妇以低压力水平为主,而双手灵活型孕妇中高压力水平更为常见。来自Donetsk和Lugansk人民共和国的孕妇,其6-硫酸氧基褪黑素水平(平均降低35.1%)和性激素水平(降低12.6%)较Rostov州孕妇显著偏低。在侧化表型的分型中,双手灵活型孕妇表现出最高水平的应激实现类激素。多因素分析(决策树模型)显示,在Donetsk和Lugansk人民共和国孕妇的压力耐受性形成机制中,循环系统适应潜能、褪黑素水平、性激素与应激释放类激素水平、体质指数和年龄占据主导地位。而在Rostov州孕妇中,影响因素的优先顺序为应激反应实现类激素水平、情境性焦虑水平以及血液系统指标。
结论。在双手灵活型侧化体质孕妇中,应激反应实现类激素分泌较高且褪黑素分泌降低,高压力水平的发生率显著更高,无论居住地区如何,这提示该类型相比极端的右侧化和左侧化表型更易受压力影响。来自Donetsk和Lugansk人民共和国的双手灵活型孕妇高压力水平发生率(高1.8倍)较Rostov州孕妇更高,表明在迁居前长期居住在战区的孕妇在压力耐受性方面下降更为明显。
