儿科重症监护室的超声导航:现实表现情况

论证。近年来,与麻醉学和重症监护中超声成像方法的有效性和安全性有关的出版物越来越多。 这些方法允许降低侵入性操作的风险,并尽早发现危及生命的状况。然而,在医院的实践活动中推行这些方法有很大的困难。这就是本研究的基础。
该研究的目的是评估儿科麻醉和重症监护专家在日常工作中使用超声导航技术的情况。
材料与方法。我们对俄罗斯联邦65个区域的儿科重症监护室负责人进行了自愿匿名问卷调查。
结果。共有32位受访者(38.4%)作出了答复。在30%的案例中,儿科重症监护室的麻醉师和复苏专家的经验在5-10年之间。没有将超声导航技术引入科室日常工作与专家经验之间的关联。所有参与研究的专家都能昼夜使用实时超声波扫描仪。在评估使用超声导航技术提供血管通路的情况时,我们发现了有5家医院(15%)完全不使用超声导航技术,只有4家医疗机构(12.5%)在100%的病例中使用超声导航技术。在大静脉插管术中使用超声导航的平均频率为49±35.5%。有26个(81%)受访者使用超声诊断方法来评估心肌收缩功能。50%的受访者将其作为儿茶酚胺输注患者的常规检查。评估心肌收缩功能最常用的方法是Teicholz法(56%),34%的病例中使用了辛普森法。56%的受访者使用超声成像来评估肺部健康状况,但只有28%的病例中将其作为肺通气患者的常规检查。47%的病例中使用超声波诊断来评估容量状态;72%的病例中使用超声波诊断来评估脑灌注和诊断颅内高压综合征。56%的受访者使用超声导航方法筛查诊断儿童多发性创伤中危及生命的综合征,在44%的病例中,这是一项常规检查。57%的受访者认为超声诊断方法非常有效。71%的受访者认为使用这些方法可确保患者高度安全。
结论。缺乏必要的知识和实践技能是在儿科重症监护室里广泛采用超声导航方法的主要障碍。
