青少年盲肠穿孔由磁性球引起的病例报告

近年来,儿童吞食磁性异物的案例在全球范围内快速增加。特别是吞食多个磁性球时,这些磁性球可能互相吸引并损伤胃肠道壁,造成严重后果。虽然磁性异物的吞食多见于6个月至4岁的儿童,但在青少年中较为少见。病例描述:一名 13岁男孩 因腹痛前往医院急诊科。病史: 入院前一天出现多次呕吐,但排便正常。以疑似急性阑尾炎收治入院。检查:临床检查及腹部超声排除急性阑尾炎; 腹部X线检查显示盲肠区域有多个高密度金属异物。进一步询问得知,患者在 2周前 吞下了几十个磁性球。诊断性腹腔镜检查:发现由回肠段和大网膜组成的炎性包块,与盲肠穹窿紧密贴附;分离包块后发现盲肠穹窿壁穿孔。手术治疗:转换为小切口开腹手术,通过右髂窝小切口进入腹腔;从穿孔处移除部分异物(18个磁性球),其余磁性球迁移至结肠;术中X线检查显示异物位于升结肠的单个团块;修补盲肠缺损并切除受影响的大网膜。术后管理:通过监测粪便及X线对磁性球迁移情况进行追踪;第3天的控制性X线检查显示磁性球停留在结肠肝曲区域;通过结肠镜检查移除剩余的15个磁性球。患儿康复,无后遗症。磁性异物可能发生在各年龄段儿童中。在儿童不明原因腹痛的诊断中,应询问家长及儿童是否接触过磁性物体,同时考虑腹部X线检查的应用。手术治疗策略包括腹腔镜和腔内内镜,具体取决于磁性异物的位置及腹腔内并发症的存在情况。
