详细
论证。根据目前的科学数据,只有 1.1% 的美沙酮中毒患者死于医疗机构。在这方面,目前法医学对上述疾病分类的看法主要是根据未经治疗和诊断过程而死亡的尸体解剖得出的。因此,新陈代谢紊乱在急性美沙酮中毒死亡机制中的变动和作用问题,以及对此类病例的法医评估问题,在法医学中基本上没有得到探讨。
该研究的目的是对一家多科性医院收治的急性美沙酮中毒死亡病例进行法医鉴定,并确定新陈代谢紊乱在导致死亡中所起的作用。
材料与方法。病历和法医鉴定报告是这项单中心回顾性随机队列研究的直接材料。在生前化学毒理学检查和死后法医化学检查的框架内,所被纳入研究的具有美沙酮检测呈阳性的观察结果是通过尸体生物环境分析确认的。皮尔逊卡方(χ2)检验用于最终解释结果和评估其统计意义。
结果。对病例的临床和法医数据进行比较分析后发现了,死前和死后确定的大多数死因是一致的,主要的死亡机制是一个综合病理过程,即肺水肿和脑水肿。死因不一致的案例主要是由于次要病理在死亡机制过程中占据首位。
结论。根据所获得的数据,我们得出了一些具有实际法医和临床意义的主要结论和相关结论。最重要的结论是,在所有病例中,美沙酮中毒的死亡结果都与严重缺氧时新陈代谢紊乱的程度有关。病人的预期寿命取决于强化治疗(呼吸机、输液治疗、升压药支持、正性肌力药支持、新陈代谢紊乱纠正)的效果,其特点是随着住院时间的延长,次要病理变化排在首位。研究发现了,在治疗这类病人时,使用纳洛酮(Naloxone)解毒剂治疗和不提供呼吸支持的观望方法是不合理的。
