论证。尿失禁综合征会恶化老年人的心理状态,增加发生焦虑、抑郁、跌倒相关伤害的风险,导致生活质量下降,中老年人和老年人的身体活动水平下降。
研究目的是根据“水晶”和“桉树”研究的数据评估尿失禁的发病率,确定与中老年和老年尿失禁发展相关的因素,以及降低尿失禁发展风险的因素。
材料与方法。对1,007名65岁及以上的人进行随机抽样。评估的主要参数:尿失禁综合征、老年衰弱综合征、营养状况、贫血、C反应蛋白水平、功能状况、抑郁症、痴呆、慢性疾病、压缩力、身体机能水平、跌倒。随访时间为2,5年。
结果。根据“桉树”研究,在48.0%的参与者中检测到尿失禁综合征,在“水晶”研究中为41.2%。在62.4%的患者中,首次诊断出尿失禁综合征。尿失禁与慢性阻塞性肺疾病、急性脑血管病病史、感觉缺陷以及老年性虚弱综合征和其他老年综合征的患病率较高有关。改善营养和增加蛋白质摄入量导致47.7%的研究参与者尿失禁症状消失。尿失禁症状的消失与情感背景的改善有关,而认知功能的下降则与中老年和老年尿失禁的发病风险增加有关。
结论。尽管尿失禁综合征的发病率很高,但它常常未被确诊。在与患者的谈话时,提出正确的问题能够多发现30%的尿失禁病例。鉴于慢性阻塞性肺疾病、急性脑血管意外、感觉障碍、老年衰弱综合征等老年综合征患者尿失禁的发病率较高,应有针对性地询问该组患者是否存在尿失禁症状。改善营养和增加蛋白质摄入量与尿失禁症状的消失有关
