论证。多数肌肉骨骼系统疾病在儿童期即被发现,这对于防止疾病迁延发展并避免成年期劳动力丧失具有重要意义。近年来,儿童疾病监测具有重要的科研和实践意义,包括对未来组织管理任务的重新评估。
目的:比较分析2017—2023年间圣彼得堡儿童肌肉骨骼系统与结缔组织疾病的总患病率动态。
材料与方法。基于2017—2023年间的官方统计报告,对圣彼得堡及全俄罗斯0–14岁和15–17岁儿童肌肉骨骼系统与结缔组织疾病的患病率进行比较评估。患病率按每千名相应年龄段儿童计算。通过对统计汇编中所列疾病的总体及具体病种计算优势比,对其特征进行了定量分析。优势比的统计学显著性以95%置信区间不包含1为准(p < 0.05)。数据分析使用 Microsoft Office 2010(Word, Excel) 进行。
结果。圣彼得堡儿童肌肉骨骼系统与结缔组织疾病患病率维持在极高水平,呈上升趋势,并与俄罗斯其他地区的差距逐年扩大。在COVID-19大流行期间,多数具体疾病分类的患病率增长幅度超过了此前时段的增长水平。在疫情期间,圣彼得堡0–14岁儿童的优势比在反应性关节病(上升23.6%)、 变形性脊柱病(27.4%)和其他类疾病(23.0%)中呈上升趋势;而在青少年中,则在幼年型关节炎(7.9%)、各类关节病(13.3%)、变形性脊柱病(15.5%)和其他类疾病(22.7%)中也观察到优势比上升。相比之下,脊柱病变在两个年龄组中均表现出检出率下降的趋势。在评价优势比的显著性与研究结论可靠性时,应优先采用最小计量单位作为分析尺度。
结论。圣彼得堡儿童肌肉骨骼系统与结缔组织疾病的患病风险持续上升,且其优势比高于俄罗斯其他地区。这表明有必要进一步完善本地区的分级诊疗体系和现代化的预防与治疗策略,同时在俄罗斯各地区提升医疗资源的数量与质量。
