Современные представления о роли вагинальной микробиоты и ее отдельных представителей в развитии бактериального вагиноза

Аннотация
Бактериальный вагиноз (БВ), являещийся широко распространенным заболеванием среди женщин репродуктивного возраста во всем мире, связан с неблагоприятными гинекологическими и акушерскими исходами. Полимикробная природа заболевания, высокая частота рецидивов (до 80%) и бессимптомного течения (до 50%), формирование обширного пласта устойчивых к традиционной терапии бактериальных биопленок обусловливают необходимость поиска новых подходов к диагностике и вариантов лечения. Целью данной обзорной работы является обобщение исследований о роли отдельных представителей микробиоты женского репродуктивного тракта в развитии бактериального вагиноза, рассмотрение межбактериального взаимодействия БВ-ассоциированных микроорганизмов на примере биопленки, а также обзор подходов к диагностике и терапии заболевания. Вагинальная микробиота является сложной и динамической экосистемой, подверженной постоянным колебаниям на протяжении всей жизни женщины, и важную роль в поддержании состояния микробиома влагалища играет род Lactobacillus. Избыточный рост таких бактерий, как Gardnerella spp., Fannyhessea vaginae, Prevotella bivia, Mobiluncus spp., Peptostreptococcus anaerobius, Megasphaera spp. и др., опосредует развитие бактериального вагиноза, в связи с чем необходимо изучение современных представлений о микроорганизмах, вовлеченных в развитие БВ, с точки зрения как их индивидуальных особенностей и факторов патогенности, так и способности вступать в ассоциации друг с другом, в том числе посредством образования биопленок.
