Optimization of drug supply to the population as a factor of strengthening public health
Abstract
this scientific article presents the results of a study on optimizing the drug supply to the population as a key factor in promoting public health. The work is based on an analysis of the availability, range and cost-effectiveness of medicines in a number of medical organizations, as well as on a study of patient and medical professionals' satisfaction with the drug supply system. In the course of the study, the main problems of providing medicines in urban and rural settlements were identified, quantitative and qualitative indicators of the provision of necessary drugs were analyzed, and ways to improve organizational mechanisms and regulatory framework were presented. The results obtained demonstrate that an integrated approach to optimizing drug provision contributes to improving the health of the population and reduces the risks associated with self-medication and delayed receipt of necessary medications by patients.

The relationship of thromboembolic complications after arthroplasty of joints with low bone density in the context of pathogenesis and pharmacoprophylaxis
Abstract
postoperative venous thromboembolic complications remain one of the main causes of death and disability in patients undergoing total knee and hip replacement in the Russian Federation. At the same time, the high prevalence of osteoporosis and osteopenia in this cohort forms an intersection of pathogenetic mechanisms that combine bone remodeling disorders and activation of the hemostasis system. The purpose of this literature review is to systematize domestic and foreign data on the association of low bone mineral density with the risk of thromboembolic events, characterize the molecular and cellular processes responsible for this interaction, and critically analyze pharmacoprophylactic strategies recommended by Russian clinical guidelines. To prepare the article, we conducted an extended search for publications from 2005-2025 in the databases of eLibrary, RSCI, PubMed and Web of Science, as well as an analysis of the national recommendations of the Association of Orthopedic Traumatologists of Russia, the Association of Phlebologists of Russia and the Russian Association for Osteoporosis. The results demonstrate that the presence of osteoporosis increases the relative risk of symptomatic venous thrombosis after joint replacement by 60-80% in the absence of an adapted anticoagulant regimen, and direct factor Xa inhibitors are comparable in effectiveness to low-molecular-weight heparins, but safer for bone metabolism. Comprehensive consideration of densitometric parameters when choosing thromboprophylaxis is a promising area of personalized orthopedic care.

New evidence on the association between androgen deficiency and the development of chronic heart failure in middle-aged men: pathophysiological mechanisms and clinical consequences
Abstract
in recent years, there has been a steady increase in cardiovascular diseases, especially among men, which can often be associated with changes in the endocrine system, including the development of age-related androgen deficiency. Testosterone, as a key male sex hormone, plays an important role in the regulation of metabolic and inflammatory processes, and the contractility of the myocardium. However, the mechanisms of the relationship between testosterone deficiency and the development of diseases of the cardiovascular system are still poorly understood, especially in the context of structural and functional changes of the heart. The purpose of this work was to study the effect of androgen deficiency on the development of secondary cardiomyopathy in middle-aged men. The results of the study showed that in middle-aged patients, a decrease in testosterone levels leads to structural changes in the myocardium and increases the risk of developing chronic heart failure.

Features of the use of antihypertensive drugs in patients with metabolic disorders
Abstract
the prevalence of obesity is increasing. Obesity is often combined with hypertension and can cause or worsen it. Some antihypertensive drugs may have undesirable effects on metabolic and hemodynamic disorders that link obesity and hypertension. Diuretics and beta-blockers have undesirable effects, while calcium antagonists are metabolically neutral, and angiotensin converting enzyme (ACE) inhibitors and angiotensin receptor blockers (ARBs) can increase insulin sensitivity. The renin-angiotensin system in adipose tissue is involved in the development of hypertension, and sodium retention plays a central role in the development of hypertension associated with obesity. Therefore, an ACE inhibitor or diuretics should be considered as first-line antihypertensive therapy for a combination of obesity and hypertension.

Options for ensuring access to medical products in emergency situations
Abstract
emergencies include natural disasters, military conflicts, and mass gatherings of people. They create significant pressures on healthcare due to a possible increase in demand for medical services. Pharmacy workers play a key role in the healthcare system by providing access to essential medicines. Disruption of drug supplies can delay medical care and worsen treatment outcomes. To find solutions for pharmaceutical care for the population in emergency situations, a keyword search was conducted for published articles in Russian and foreign databases. The key step is to prepare standards and train employees to act in emergency situations. To solve logistical problems and improve transport accessibility in pharmaceutical provision, it is necessary to create a single database for public and private pharmacies integrated with EMIAS for timely deliveries to medical institutions and the introduction of mobile pharmacies. In order to provide medicines to the population, it is necessary to limit the uncontrolled purchase of medicines during an emergency, develop a system of distribution points with a supply of medicines for emergency situations and clearly define the criteria for the emergency use of accelerated approved drugs.

Diagnostic laparoscopy for abdominal peritonitis
Abstract
the article is devoted to the study of diagnostic laparoscopy for abdominal peritonitis. The relevance is justified by the requirements of improving the accuracy of diagnosis and reducing the traumatic nature of surgical interventions in acute peritoneal inflammation. The novelty lies in the comparative analysis of the outcomes of endoscopic and open access, taking into account the stability of hemodynamics and the stage of inflammation. Within the framework of the work, the diagnostic possibilities of laparoscopy are described, options for therapeutic manipulations through trocars are presented, and the access selection algorithm is substantiated. The indicators of diagnostic accuracy, the frequency of postoperative complications, and the duration of hospitalization were studied according to domestic and foreign sources. Special attention is paid to the criteria for selecting patients and the conditions for conversion to laparotomy. The work aims to identify effective practices for the use of diagnostic laparoscopy in various forms of peritonitis. To solve this problem, comparative analysis of publications, systematization of recommendations, synthesis of clinical data and development of an algorithmic decision-making scheme were applied. The works of A.S. Harutyunyan, O.V. Galimov, U.K. Ballehaninna et al. have been studied. In conclusion, the impact of the chosen strategies on the speed of recovery of patients and the safety of treatment is described. The article will be useful for surgeons, researchers of minimally invasive technologies and heads of surgical departments.

Analysis of the clinical efficacy of homeopathic treatment in patients with chronic sinusitis
Abstract
goal: to evaluate the clinical efficacy and dynamics of quality of life in patients with chronic sinusitis using individualized homeopathic therapy. Materials and methods. The study was conducted at the Republican Clinical Hospital in Makhachkala from January 2022 to March 2025. The follow-up included 20 adult patients with a confirmed diagnosis of chronic sinusitis lasting more than 12 weeks (ICD-10: J32.9). All patients received a course of classical homeopathic therapy prescribed by doctors with at least 3 years of experience. The severity of symptoms was assessed on a numerical scale (NRS), and the quality of life was assessed on the SF-36 questionnaire. The follow-up period was 24 months. The analysis included assessments of the dynamics of symptoms, quality of life, and frequency of use of traditional medications. For statistical analysis, multiple linear regression with imputation of missing data using the Rubin method was used. Results. During the first 3 months, there was a marked decrease in the intensity of symptoms (the average NRS score decreased by 2.1 points), which persisted in the subsequent period. After 2 years of therapy, 85% of patients showed a steady improvement in their subjective well-being, and the frequency of using traditional medicines decreased by 47%. The quality of life in the psycho-emotional component significantly improved (p < 0.05), while the physical component did not show significant changes. No side effects were recorded. Conclusions. A two-year follow-up demonstrated the potential effectiveness of homeopathic therapy as an additional approach in patients with chronic sinusitis, especially in the case of insufficient effect of traditional treatment. The data obtained allow us to recommend further study of the mechanism of action and the expansion of indications for the use of homeopathic remedies in otorhinolaryngology.

The image of Vladivostok entrepreneur: a comparative study of practicing and prospective businesspeople
Abstract
the article presents research findings on the image of Vladivostok entrepreneurs among established (N=51) and aspiring entrepreneurs (N=52). The study's relevance stems from Vladivostok's unique position and the growing importance of small and medium-sized enterprise development in current economic conditions. The research aimed to identify perceptual differences in entrepreneur representation between these groups. The methodology incorporated survey techniques, verbal associative experiments, and V.P. Serkin's semantic differential. Data analysis employed frequency analysis, categorical classification, factor analysis, and Fisher's exact test. Key findings revealed motivational differences: current entrepreneurs emphasized pragmatic aspects (adaptability, necessity), while future entrepreneurs focused on status and creativity. Factor analysis identified common image components ("Professional," "Proactive Leader") and a unique regional-specific factor ("Adventurer"). The practical significance lies in applying these results to entrepreneurship support programs and image policy development. The findings offer value for government agencies, business accelerators, and educational institutions. Future research should explore intergenerational differences and regional entrepreneurship specifics.

Psychological well-being of teachers with different types of professional identity
Abstract
the study examines the psychological well-being and professional identity of teachers. The relevance of the topic is driven by increasing psycho-emotional demands in teaching, which heightens the risk of burnout and reduces the overall effectiveness of the educational process. The work employs the eudaimonic approach (C. Ryff) to analyze psychological well-being and L.B. Schneider’s concept of professional identity. The study aims to identify differences in psychological well-being characteristics depending on the type of professional identity. The research involved 51 college teachers from Vladivostok, including 39 women and 12 men. The Kruskal-Wallis H-test was used to assess differences, as it is applied for comparing two or more samples based on a given criterion. The results revealed significant differences in psychological well-being levels among teachers with different types of professional identity (p ? 0.01). Teachers with a positive achieved identity demonstrated higher well-being indicators, whereas diffuse and crisis identities were associated with low well-being levels. The findings contribute to occupational psychology and educational psychology, expanding the understanding of factors influencing teachers’ professional resilience.

Analysis of images of anti-crisis state policy measures among the youth of the Far East
Abstract
in crisis situations, different people prefer different ways of behaving. Differences in reactions are partly due to differences in the perception and interpretation of a particular context. In crises that affect a large part of the population and sometimes involve drastic lifestyle changes, subjective factors that influence the formation of certain ideas and images play an important role. The psychological patterns and structures of public policy images in the minds of Russian youth are of the greatest interest, since, on the one hand, young people have considerable potential in building the country's future, on the other, the effectiveness of government anti-crisis policy measures depends on the socio-psychological situation in society. Over the past few years, the world has changed quite a lot towards increased uncertainty. The loss of a sense of security, distrust of the world, emotional tension in society, polarization of opinions and division into "camps" and so on lead to the fact that the planning horizon of modern youth may be much shorter than it was ten years ago. The alarming mood in society and the return in the social context can be partially compensated by government policy to counteract crises, but in order to carry out effective measures, it is necessary to rely on feedback from society and take into account the socio-psychological climate. We conducted a study to study the structural and substantive characteristics of anti-crisis policy images in the minds of Russian youth living in the Far East.

Digital environment that stimulate the manifestation of personal aggressiveness
Abstract
in the context of the rapid expansion of the digital space and its integration into the educational and social environment, aggressive behavior in online communication is increasingly being recorded. This phenomenon is becoming particularly relevant for educational psychology, as it requires a rethink of the factors contributing to the formation of personality aggression in the digital environment. The article provides an overview of modern domestic and foreign studies devoted to the study of the relationship between Internet and smartphone addiction, decreased empathy, increased anxiety and aggressive behavior. A special role of empathy is highlighted as a buffer mechanism that reduces the risk of aggression with a high level of digital engagement. Personal predictors of aggression, such as neuroticism, impulsivity, and cognitive distortions, as well as psychophysiological and cognitive aspects that exacerbate destructive behaviors, are also considered. The influence of the phenomenon of online disinhibition and digital disindividuation on a decrease in self-regulation on the web is substantiated. Finally, the directions of psychodiagnostics and prevention of digital aggression in the educational environment are proposed. It is concluded that there is a need for an integrated approach to the study of digital aggression, taking into account both individual and environmental components, which is especially important in the context of psychodiagnostics and student support.

Professional identity of employees and conflict management strategies
Abstract
the article analyzes the relationship between the type of professional identity of employees and their strategies of behavior in conflict situations. The work covered both theoretical and empirical aspects of this problem, which allowed for a comprehensive approach to its study. Special attention is paid to various styles of conflict behavior and their dependence on the level of professional identity formation. Empirical research has confirmed the hypothesis that there is a significant relationship between the level of professional identity and the preference for certain behavioral strategies in conflict. It has been found that employees with an established professional identity are more likely to resort to cooperation and compromise, while diffuse and premature types of identity correlate with the choice of competition and avoidance. This indicates that the formed professional identity not only contributes to personal stability, but also has a positive impact.

Features of stress resistance in high-child mothers
Abstract
the article describes the results of a comparative analysis of the level of stress tolerance among mothers with many children and those with few children. The study sample consisted of 142 women, of whom 37 were mothers with many children (three or more children) and 105 were mothers with few children (one or two children). Using the method "Test for determining the level of stress tolerance of personality" by I.A. Usatov, it was found that women with many children have a significantly higher level of stress tolerance compared with single and two-child women. Mothers with many children with different attitudes to motherhood (motherhood is happiness and joy; it is a lot of work; it is a great responsibility) showed no significant differences in the level of stress tolerance, while mothers with few children showed a higher level of stress tolerance when they perceive motherhood more positively, that is, as happiness and joy, rather than as a lot of work or as a big responsibility. Mothers with many children are more likely than mothers with few children to run their own businesses and are less likely to be employed. This fact reflects the fact that mothers with many children, who are heavily burdened with household work, are also more motivated than mothers with few children to contribute financially and materially to their families.

Role of self-organization and self-regulation in the organizational culture of teachers of modern educational institutions
Abstract
modern policy in the field of education and upbringing pays great attention to ensuring a high level of work and educational activities, the foundation of which is the self-regulation and self-organization of each teacher. The socio-emotional competence of the teaching staff has a direct impact on the effectiveness of the educational process, the work of colleagues and the creation of a work- and learning-friendly environment in an educational institution. Consequently, the hierarchical set of educational activities is based on an organizational culture that promotes the control and coordination of teams, as well as their integration into a single value system, the mechanism of which functions clearly and harmoniously not only in compliance with job descriptions, certain norms and recommendations, but also if teachers have skills in self-organization and self-regulation. The quality of management, professional experience and maturity of the team are closely correlated with the organizational culture of the institution, as compliance with the latter contributes to the formation of beliefs, principles and priorities of teachers and students. Based on the results of the research, the following conclusions were formulated: the existing types of cultures are chosen depending on the mentality of the people, where it is incorporated – in Russian realities, the synthesis of cultures of the organization is most relevant, in which the "role-playing" is dominant.

The relationship between cognitive errors and personality traits in students
Abstract
cognitive distortions are systematic deviations in cognitive processes that influence an individual's decision-making and behavioral patterns. These biases can manifest themselves in various forms among students, affecting different aspects of their academic activities and personal lives. This, in turn, can have a significant impact on academic performance, interpersonal relationships, and the formation of personal qualities. Understanding the nature of cognitive distortions and their impact on students' decision-making is extremely important for both students and faculty, as this knowledge can be used to develop more effective educational strategies and methods of student support. We have identified a link between inflated self-esteem and the cognitive distortions of “maximalism,” “categoricality,” and “self-confidence,” as well as between high levels of anxiety and the cognitive distortions of “catastrophizing,” “trusting critics,” and “personalization”.

Attitudes towards interethnic marriages in modern society
Abstract
the modern world is full of various new trends. One of them can be considered a change in attitude towards interethnic marriages. Such marriages are not only increasing in number, but also becoming more common, not causing rejection and negative perception among a large number of people. At the same time, there was, is and, probably, may remain in the future, a part of the population that has a negative attitude towards such marriages. Often, the acceptance of interethnic marriages is explained by certain factors: religious, cultural, social, political. In a world where there is an opportunity to learn about other cultures, be mobile, not depend on the opinions of others, and even families, interethnic marriages are becoming more real. This trend is especially evident among young people living in territories with clearly expressed polyculturalism, multireligiosity, egalitarianism. In territories of national and religious majority (for example, national republics and territories), the attitude towards multiethnic unions is more reserved, and sometimes clearly negative. We believe that the personality itself, its attitudes, values and views on life play a major role in the acceptance or rejection of polyethnic marriages. A personality that shows readiness for family life and tolerance is more capable of creating such a union and living in it, going through all the stages of the life cycle. At the same time, nationality will be only one of the components of family life. In order to study the attitude towards interethnic marriages, we conducted a study. In general, we received confirmation of the main trends in people's assessments of such unions. However, it should be taken into account that the territory where the study was conducted (Khabarovsk Krai) can be classified as accepting new forms of marital relations.
